Getting Ready for Your First Visit

 

Woohoo! You’ve been scheduled. Now, let’s make sure you have everything you need for your first appointment.

Please Note: Failure to bring each of these items may result in your appointment being rescheduled.

Referrals
If your insurance plan requires a referral for you to be seen by us, we will work with you to obtain the referral from your primary care provider. However, please note that it is your responsibility to make sure the referral has been both received and processed by us prior to arriving for your appointment.

Payment Policy
Patients are responsible for payment of services rendered. If you have insurance, it is your responsibility to check with your insurance carrier whether the service you may be getting scheduled for is covered in your agreement. Certain services may require you to pay a copayment or share a percentage of the cost. We rely on you to be familiar with the individual contract you have with your insurance carrier.

If your insurance information or coverage changes at any point during your time as our patient, it is your responsibility to let us know so pending charges are billed appropriately. If we are not notified and the window to resubmit your claim has passed, the balance will become patient responsibility.


 

01 — New Patient Packet

Click here to access our new patient packet. You’ll need to fill this out and bring it with you to your first appointment before you can be seen. If you are not able to complete this packet and bring it with you, please plan to come in 30 minutes before your scheduled appointment time to ensure timely service.

02 — Insurance Card(s)

Please bring all of your insurance card(s) to every visit so we can stay up to date and make sure your claims are being paid. If your insurance coverage ever changes during your time as our patient, please let us know as soon as possible to avoid any confusion.

03 — Photo ID

As a new patient, you cannot be seen in the office unless you bring in a valid photo ID with you to your visit(s).

04 — Current List of Medications

Please take a moment before your appointment to write down a list of the medications you are currently taking. This will help your doctor create the most appropriate plan of care.
